About Patient Navigation
Patient navigators guide patients through the complexities of the health care system and overcome barriers that prevent them from getting the care they need.
We offer patient navigation to guide patients in the community through their health care system by connecting them to health care providers and other community resources. The navigator meets with you one on one, formulates a plan, document your goals, and help you make phone calls. We provide instructions for you regarding your specific healthcare needs and encourage you to achieve optimal health.
